To modify or create forms, click on Tools -> Database and Forms -> Edit Contact Details Forms.  The Form Editor will pop up.  As with all windows within ContactTalk, the Form Editor can be moved to any area of the screen simply by dragging it to the desired location.

The Form Editor will then be displayed.
